Make A Referral
Physicians Referring for Skilled Services
Referrals to skilled care can be made easily by phone, fax or the Allscripts ECIN network. Please provide as much of the following information as possible:
- Patient's name, date of birth, address and phone numbers
- Patient's representative name and phone number, if applicable.
- Physician's name, phone and fax number
- Orders for service
- Diagnoses and pertinent past medical history
- Patient's medical insurance information
The patient or representative will be contacted for any missing information.
1) By Phone: 847-256-1705.
A written order request will be faxed to the physician for signature. After hours, ask for the on-call nurse who will promptly return your call.
2) By Fax: 847-512-0962.
Please fax the information above along with a signed order by the physician if available.
3) Allscripts ECIN Network.
Care in the Home is a member of the Allscripts ECIN electronic referral network. If you are a member, please search for us under Care in the Home in the provider database. If you are interested in becoming a member, please contact Allscripts at http://www.extendedcare.com/.
Referrals to Personal Care Services
If you know someone who might be interested in a home caregiver, please call us at 847-256-1705 and ask for the Admissions Department.
