Nurse Case Management

Our RN case managers meet with you and your family to develop a care plan to meet your specific needs. The RN stays involved throughout the duration of each case to ensure the quality of care and to facilitate any changes in personal care needs. Our RN case managers:
  • Are available 24 hours a day, seven days a week
  • Open and closely monitor each individual case
  • Do a comprehensive client assessment
  • Conduct a thorough home safety evaluation
  • Create medication profiles and coordinate with your primary physician
  • Train and supervise staff to meet the specific healthcare and personal needs of each and every client

Our Care Management philosophy stresses our clients' ability to retain their independence by living in their own homes and retirement communities; and remaining in their own communities. One of the tools we use is a "Home Safety Evaluation". One of our qualified nurses will visit you in your own home, and evaluate items such as the ones shown below. Then, we shall make recommendations based upon your answers and our years of experience.

If you answer yes to any of the questions below, you should consider discussing the need for assistance at home with your physician, family or Care in the Home. Please contact Care in the Home for more information or further assessment of your need.

  1. I live alone.
  2. My physical condition does not permit me to care for myself and do the things I was able to do before my hospitalization.
  3. My family is unable to care for all my needs.
  4. My doctor has prescribed a special diet.
  5. I have difficulty selecting and preparing my own meals.
  6. I don't understand my disease and how to control it.
  7. I have more than one medical problem.
  8. I take many prescription medicines.
  9. My doctor has prescribed some new medicines which I have never taken before.
  10. I had an operation during my recent hospital stay.
  11. I have an unhealed wound and I need assistance with dressing changes.
  12. I have other special medical needs and would feel more confident if I received some additional teaching at home.
  13. My physical limitations make it difficult for me to use my bathroom at home.
  14. My physical limitations may require me to learn to use a wheelchair, walker or other assistance devices to manage at home.
  15. I have been in the hospital twice in the last six months.
  16. I have had visits to my home by a nurse or other health care professional in the past.
  17. I am homebound, making it difficult to do errands, etc.
  18. I have trouble bathing myself.
