ABOUT CARE IN THE HOME

Care in the Home was founded in 1989 by Ruth Friedman as a private-duty home caregiving agency. Since then, our organization has expanded to offer skilled Medicare services, including nursing and therapy. When Ruth decided to sell the company, she entrusted it to one of her long-time employees. For nearly seven years, Tracy Kelly worked as the Nurse Clinical Supervisor, leading with compassion, expertise, and an unwavering commitment to the well-being of both patients and staff. Her dedication not only elevated the care provided, but it also shaped the culture and values of the organization.


In 2021, Tracy took an extraordinary step; she purchased the company that she dedicated so many years to ensure that its core values—compassion, excellence, and integrity—remained at the heart of everything we do. By becoming the owner, Tracy has not only preserved the legacy of the company but also redefined its future, ensuring that both patients and employees continue providing professional expertise and ethical care, adhering to best practices in all aspects of service delivery, and treating every client with the same compassion and dedication as we would our own family.

CASE CONSULTATION AND MANAGEMENT

CASE MANAGEMENT
Care in the Home offers two types of free consultations:
ADMISSIONS CONSULTATION

Our admissions consultants are available by phone at 847-256-1705 Monday through Friday from 9 AM until 5 PM. The goal of every discussion is simply to be helpful in an honest, forthright manner. To get the most of this service, the consultant will explore the details of the client's condition and circumstances and then will discuss how our services could be helpful. If the caller is not comfortable discussing details, the consultant can provide information on our services and rates in general. This type of consultation is also available in person at our office during business hours.

CASE MANAGER ASSESSMENTS

Our clinical case managers are available to meet in the home Monday through Friday between 9 AM and 5 PM. Each assessment involves a detailed discussion of the client's condition and needs as well as a physical assessment of the client and the home in terms of functionality and safety. Recommendations are offered based on the assessment. All caregivers are supervised by the case manager. Please call 847-256-1705 and ask for the Admissions Department to schedule.

ALZHEIMER'S/DEMENTIA CARE

Caring for a client with dementia or Alzheimer's can require extra patience, flexibility, gentle but firm direction, and a calm, positive attitude.

 

The care of a person with dementia can be challenging and as unique as each and every client. Care in the Home recognizes the value of routine and consistency. We provide a case manager who works with the caregiver, client and family to solve problems and tackle challenges. Our clinical staff is also on-call 24 hours/7 days for any questions or problems our caregivers encounter.

END OF LIFE CARE

Dignity and Comfort

We have been working with hospices for the past 25 years to make the wish of dignity and comfort in the final phase of life a reality for our clients. Our staff coordinates closely with the hospice team to meet all needs. With good advanced end-of-life planning, hospice care and a personal caregiver, spending the final days in the comfort of your own home is a near certainty.

 

Hospice care is provided as a benefit through Medicare and most private medical insurances and offers the services of a registered nurse, bath aide, social worker, chaplain and volunteers. However, the hospice benefit does not provide 24-hr care except in cases of crisis or short-term respite. Typically, a hospice nurse visits once or twice a week, a bath aide twice a week and then other services only as needed. Families will often supplement this wonderful care with a personal caregiver for extra bath visits or as much as 24-hr care to ensure that the patient has all the help they need to remain in their home.
